It is typically performed when a root canal treatment has failed to completely heal the tooth or when infection reoccurs at the root end. Also known as root-end surgery, this procedure helps save the natural tooth and prevent the need for extraction.
When infection or inflammation persists after a root canal therapy, it may be due to a hidden fracture, narrow or curved root canals, or small accessory canals that were not completely cleaned or sealed. In such cases, an apicoectomy allows the dentist or endodontist to directly access the problem area, clean it thoroughly, and seal the root tip to prevent further infection.
The procedure is performed under local anesthesia. A small incision is made in the gum near the affected tooth to expose the underlying bone and root tip. The endodontist then removes the infected tissue and the tip of the root. After cleaning the area, a small filling is placed at the end of the root canal to seal it. The gum is then stitched back in place to allow healing. The procedure usually takes about 30 to 90 minutes depending on the location and complexity of the root structure.
Apicoectomy is performed under local anesthesia, so the procedure itself is not painful. After the surgery, some tenderness or swelling is normal but usually mild. Your dentist will provide aftercare instructions to keep you comfortable and reduce the risk of infection or complications during healing.

Recovery from an apicoectomy is generally quick. Mild discomfort and swelling may occur for a few days following the surgery. Pain can be managed with over-the-counter painkillers or medications prescribed by your dentist. Ice packs may help reduce swelling. Most patients return to normal activities within a day or two. The surgical site may take a few months to fully heal internally.
