It involves the removal or repositioning of excess skin, fat, and muscle around the upper or lower eyelids. This surgery not only improves the aesthetic appearance of the eyes by giving them a more youthful, refreshed look but can also enhance vision when drooping eyelids obstruct the visual field.
As people age, the skin loses elasticity, and muscles supporting the eyelids may weaken, causing the eyelids to stretch and excess fat to accumulate. This can result in droopy upper eyelids, under-eye bags, or puffiness that makes one look older or tired. In some cases, the sagging skin can interfere with peripheral vision, especially in the upper field. Blepharoplasty addresses both cosmetic and functional concerns, restoring a more alert and rejuvenated appearance while improving vision in cases of obstruction.
The ideal candidate for blepharoplasty is someone in good overall health, typically over the age of 35, who is bothered by drooping eyelids or persistent under-eye bags. In younger patients, inherited traits such as under-eye puffiness or hooded eyelids may also be corrected with eyelid surgery.
Blepharoplasty is typically performed under local anesthesia with sedation or general anesthesia, depending on the complexity and whether it involves both upper and lower eyelids. For upper eyelid surgery, the surgeon makes a small incision along the natural crease of the eyelid to remove or reposition excess skin and fat. In lower eyelid surgery, the incision is usually made just below the lash line or inside the eyelid to remove fat deposits or tighten the skin. The incisions are closed with fine sutures that leave minimal scarring, and the procedure usually takes between one to two hours.

Recovery from blepharoplasty is relatively short and straightforward. Most patients can resume normal activities within 7 to 10 days, although swelling and bruising around the eyes may persist during the initial healing phase. Cold compresses, prescribed medications, and proper post-operative care help reduce discomfort and support healing. Patients are usually advised to avoid strenuous activities and direct sun exposure for a few weeks to ensure the best cosmetic outcome.
While blepharoplasty offers long-lasting results, it does not stop the natural aging process. The outcomes of upper eyelid surgery can last for 5 to 10 years or even longer, whereas results from lower eyelid surgery are often more permanent. Maintaining a healthy lifestyle, protecting the skin from sun damage, and following a good skincare routine can help prolong the benefits of the surgery.
Blepharoplasty is considered a safe and effective procedure when performed by a qualified and experienced plastic or oculoplastic surgeon. Like all surgeries, it carries some risks, including infection, dry eyes, blurred vision, difficulty closing the eyes, or asymmetry.
