DaycareTESA is a needle-based technique where a fine needle is inserted into the testis to aspirate sperm. It is minimally invasive and usually performed under local anesthesia. Micro-TESE, on the other hand, is more advanced and involves using an operating microscope to carefully open the testicle and identify small areas where sperm may still be produced. It is often more successful in cases of non-obstructive azoospermia, where sperm production is severely impaired.
TESA is commonly advised for men with obstructive azoospermia, where sperm is produced but blocked from reaching the semen. Micro-TESE is recommended when the cause is non-obstructive, meaning the testicles are not producing enough sperm naturally. These techniques offer hope to couples struggling with male infertility and enable them to pursue biological parenthood.
TESA is a short, outpatient procedure typically done under local anesthesia. A small needle is inserted through the scrotal skin into the testis, and fluid or tissue is aspirated. Micro-TESE is more intricate and requires general or spinal anesthesia. A small incision is made, and under a high-powered microscope, the surgeon inspects the seminiferous tubules to find even rare sperm-producing areas.

Recovery from TESA is quick, and most patients return to normal activity within a day or two. There might be slight swelling or discomfort, which is easily managed with medications. Micro-TESE may require a slightly longer recovery due to the nature of the procedure. Patients are advised to avoid strenuous activities and follow the doctor's care instructions to ensure proper healing.
While success rates vary depending on the cause of infertility, Micro-TESE generally yields better sperm retrieval rates in non-obstructive cases. The sperm collected is used for ICSI (Intracytoplasmic Sperm Injection), where a single sperm is injected directly into an egg. Many couples have achieved successful pregnancies using sperm retrieved through TESA or Micro-TESE.
Undergoing sperm retrieval procedures can be emotionally challenging for men. It's important for couples to receive emotional support and counseling during the fertility journey. Understanding the process, having realistic expectations, and maintaining open communication with the medical team can make the experience more manageable and positive.
